1. The employee or student who requires respirator use, or their supervisor will notify the Chemical Hygiene Officer (CHO).

   The CHO will provide the employee with the OSHA Respirator Medical Evaluation \[Appendix C to Sec. 1910.134\]

3. The employee will complete the form and schedule an appointment with the Licensed Health Care Professional (LCHP) through EMU's Health Services to review the form and complete a Health Evaluation.
4. The LCHP will provide a written recommendation stating whether or not the employee is medically able to wear a respirator, including any limitations of use and any recommendations for future medical evaluations if necessary by completing Appendix O(3).

   The employee will schedule an appointment with the CHO for fit testing and bring the recommendation from the LCHP \[Appendix O(3)\].

6. The CHO will conduct the fit test using the isoamyl acetate option in OSHA Fit Testing Procedures 1910.134, Appendix A and document the fitting on the Fit Test Record.
