Appendix O(4) - Respiratory Protection Program Procedures for Entry


This document summarizes the process for entry of new employees or students to the respiratory protection program. Full information on the scope, responsibilities, and technical details of the program are available in EMU's Chemical Hygiene Plan, Appendix O.

  1. The employee or student who requires respirator use, or their supervisor will notify the Chemical Hygiene Officer (CHO).
  2. The CHO will provide the employee with the OSHA Respirator Medical Evaluation [Appendix C to Sec. 1910.134]

  3. The employee will complete the form and schedule an appointment with the Licensed Health Care Professional (LCHP) through EMU's Health Services to review the form and complete a Health Evaluation.
  4. The LCHP will provide a written recommendation stating whether or not the employee is medically able to wear a respirator, including any limitations of use and any recommendations for future medical evaluations if necessary by completing Appendix O(3).
  5. The employee will schedule an appointment with the CHO for fit testing and bring the recommendation from the LCHP [Appendix O(3)].

  6. The CHO will conduct the fit test using the isoamyl acetate option in OSHA Fit Testing Procedures 1910.134, Appendix A and document the fitting on the Fit Test Record.


Records
The CHO will maintain records of the LCHP recommendation [Appendix B] and Fit Test Record.
The Health Services Director will maintain records of the OSHA Respirator Medical Evaluation and LCHP recommendation [Appendix O(3)].
